The latest version of the SiteCatalyst template, both the Standard template and the AppMeasurement template, assume the E-commerce extension is enabled. When the E-commerce extension is not added to the profile the tag will error out and not send the request.
Many of our tags are built this way and assume the E-commerce extension has been added to the profile. So it is always a good idea to add the E-commerce ext to the profile even if you are not tracking E-commerce data.
If you add the E-commerce Extension into the profile it will fix the issue you are seeing. I tested this on the site you provided in your support ticket and saw all instances of SiteCatalyst firing.
NOTE: There is an update going out for the SiteCatalyst tag that removes the requirement for the E-commerce extension. This update will be released next week.
