What does status of "loaded" mean in Web Companion?

I have a tag - two simple image tags inside a Tealium Pixel Container - which is giving the status of "loaded" instead of "OK" in the web companion. What does this mean, so I can troubleshoot the issue with the tag.

Hi Stephen, Loaded means that the template was loaded but no call was sent. You should see an "OK" response if the tag was actually fired. It sounds like the pixel code input into "Pixel 1" is incorrect. If you are comfortable pasting the URL call here I can debug for you. Otherwise, contact your Account Manager and they'll have this researched by a Solutions Engineer.

Interesting, when I setup a basic pixel container loading with this URL I see the call occurring and the redirect happening. Web Companion shows the "OK" status too. Just curious, are you seeing the expected call in the network logs? Seems like you are but just want to be sure. If you have FireBug open, are there any Console warnings or errors?

I am gettting some weird things in Firebug. I probably need to speak in generalities to avoid company-sensitive info (let me know if I should take this offline, I'll follow up with my rep.) But, basically, when I opened new tags or loadrules, it inserted my email address in some fields. Now, in Firebug, it's doing a call to http://www.[mysite].com/[my email address], which I've never seen before and is very strange. I've tried Chrome and FF, both with clean cache and cookies, so I'm stumped.
